Carly is a Community Link Worker for Wansbeck PCN.

A Community Link Worker provides full social prescribing/link worker support to our patients who have non-medical issues affecting their wellbeing, by offering advice, signposting and making outward referrals. They also facilitates groups and activities within the surgery and wider community.
