Community Link Workers 

The Community Link Worker service provides full social prescribing/link worker support to our patients who have non-medical issues affecting their wellbeing, by offering advice, signposting and making outward referrals. They also facilitate groups and activities within the surgery and wider community. To be referred to the service, please as a member of the practice team.

DWP Patient Advisory Service

The DWP Patient Advisory Service can provide support with a range of issues including claiming benefits, work & health, employability, work capability assessment and more. To access the service, please as a member of the practice team.

Community Midwives

We have a midwife who will look after you during your pregnancy and after you have had your baby.  We hold weekly ante-natal clinics (Tuesday).

 

Community Nurses

The District Nursing Team carry out a wide variety of duties such as:

  • Drug Administration
  • Simple and complex wound care
  • Continence assessment
  • B12 injections
  • Catheter care
  • Pressure care assessment
  • Ear Irrigation (surgery clinics only)
  • Palliative care
  • Caring for complex deteriorating patients

District nurses run clinics in the surgery but also visit patients who are housebound and unable to get out. To book an appointment or home visit, please call 01670 536211. 

When carrying out house calls they are unable to specify exact dates and times that they will be calling as they expect you to be home, however, they do recognise that some housebound patients do have to visit hospitals and day centres at times; If this is the case, when organising your home visit with the nurse, please let them know any relevant information with regards to this.

Health Visitors

A health visitor is a registered nurse who has received training particularly related to babies, children and pregnant women. Their role is to provide families with children under five years old with support and advice around the general aspects of mental, physical and social wellbeing.

 

Podiatry

We have podiatry clinics available on a Thursday. Please contact Podiatry on 01670 859049 to book an appointment. 

The podiatry service includes:

  • Foot care and advice
  • Diabetic foot checks
  • Nail Cutting
  • Nail Surgery

Housebound patients can arrange home visits for a podiatrist to call out. Please contact Podiatry on 01670 85904. 

 

Health Visitors

A Health Trainer can support you with a range of lifestyle aspects including diet advice, weight management, smoking cessation, healthy eating and alcohol awareness. Please ask the Reception Team for a referral.
